Sickle cell disease is particularly common in people with an African or Caribbean family background.
Social Integration With Sickle Cell
01
We will use a non-clinical based approach to support service users with sickle cell in the community and to measure the impact of social factors on their ill health and wellbeing
02
We will sign-post to the appropriate community services or agencies that support service users with sickle cell and associated ill health, isolation and loneliness.
03
For an integrated approach, we will engage with the community and volunteering sector addressing issues and inequalities surrounding none physical and ill health in the BME community. We aim to create a service that fit around our community and not people around our services.
04
Working in partnership with the community and organisations will be vital to the success of this project and integrating these major stakeholders in shaping and delivering a robust, comprehensive and holistic front door partnership service; that is easy to access, open and transparent.
